When placing an order, please supply your purchase order number, catalog numbers, and description, quantity desired, “Ship to” and “Bill to” address, and the name and telephone number of a person to contact if clarificaiton is required. A confirming order, mailed after a telephone order, must be marked “CONFIRMING” to avoid duplication.
